1. Did a patient education representative give you instructions on how to care for yourself after your illness or operation?
I did receive vague instructions in regard to my new diagnosis of diabetes, , and hypertension. However, it was not until my follow-up appointment when my doctor recommended diabetic and nutrition classes my health insurance covers. Those classes were very helpful. I learned how to manage my diet, adequate portions, and how to read nutrition labels. I also made new friends who are an additional support group, lost weight and my lab work is improving the doctor says.
2. Did a health care professional, pharmacist, nurse, doctor, or elder counselor advise you on your medication, diet, or exercise?
Yes. At the hospital, the doctor and nurses explained everything to me the hospital. Then, once I picked up my medications from pharmacy the pharmacist went over the medication thoroughly. I was also told that someone would come to my house and go over everything again with me.
3. Who assisted you at home after your illness or operation?
The hospital arranged home health with a nurse for diabetes education. The nurse showed me how to take my own blood pressure and blood sugar readings. She went over a diet plan to help me with my salt and sugar in take. She also helped me to set up a mild daily exercise routine.
4. Do you know of any assistance services, i.e., food, transportation, medication, that would help you stay in your home as you get older?
I really don’t know of any, but I know a lot of people that could be good resource for me to get some information. My daughter and son-in-law are both in the medical field. They helped me a lot during my illness and continue to be very involved with my health care. They go with me to appointments and make sure I am compliant with my health care management. I could have benefited from extra resources provided in regard to transportation, meals and caregiver since I am alone. But, they never provided me with those sources.
In conclusion, patient education is crucial to an individuals help. For example, Mr. Mendoza was overwhelmed when he was first diagnosed with diabetes, hypertension and coronary artery disease. In addition, he was scared. He has no medical background. This was all very new for her. He said they were talking in medical terminology which is a foreign language to me. But, one nurse really took the time to sit down and acknowledged my concerns. Although, Mr. Mendoza has a support system and a daughter in the medical field who can assist her; the health care professionals should have acknowledged her health literacy and improve their education skills. are better able to understand and manage their own health and medical care throughout their lives. Health care professionals communication is key element of patient education and is often used in conjunction with other teaching practices.
