Jane Jenkins, the Safety Officer has become alarmed at the increase in the number of accidents.
Not only is she concerned about the human aspect, but she is worried about the loss of
productivity and the increase in insurance premiums that will inevitably follow if this trend
continues.
Since you were available, she has asked for your thoughts on the situation. Jane sees no
apparent connection between the accidents since they involve different people from various
departments where the accidents have taken place. However, the numbers indicate the potential
problem, the accident rate is increasing as the following chart indicates and there is a large
increase for October.

accidents to date. The information you wanted is set out on the next pages.
The Assignment Detail
Jane would like your ideas on the possible cause(s), prevention and how you think the workplace
could be made safer in relation to the accidents in the cafeteria area. Cost is an issue, times are
hard and orders are just as hard to win. Managers appear driven by costs and production targets.
You are also to include in your report to her, any observations surrounding general safety issues
you may observe, and using epidemiological analysis from the accident log. Include any control
measures or the suggest necessary improvements that may also aid how H&S is managed that
you can reasonably deduce from the map, accident log and information given. There is no safety
committee or formal employee consultation.
The site is a new build area, and the rush to move into the site and get production underway was
rather rushed. The factory has a busy traffic system with deliveries of raw substances and
vehicles taking away completed products. Road speeds currently conform to National Limits.
